The most common problem that we see is accidental damage. The metal housing where the volume buttons are is a weak spot and if it takes a knocn in that area it can bend inwards and damage the internal buttons. One of the buttons is constantly pushed in so you would see the icon on the LCD showing the volume control all the time.
Sometimes just wear and tear over time can cause the internal buttons to become unresponsive. This is more common on the home button.
